The largest wireless carrier in the United States is currently investigating how the data of 73 million customers (current and former) has ended up on the dark web. While AT&T decided to go with a mass reset of account passcodes and confirmed the leak, they also denied any system breach. Thankfully, the data is at least half a decade old.
